Ready to join the NYC Spine family? Our “New Patient Intake” form is provided below for your convenience. Download and complete our forms prior to your arrival to speed up the registration process on the day of your very first appointment!
In addition, patients with neck or back discomfort should also fill out the “Neck Disability” and/or “Back Disability” indices, respectively. Patients with spinal deformities (such as scoliosis or kyphosis, for example) should complete the “Spinal Deformities” questionnaire as well. These custom forms are used to help your surgeon evaluate the degree to which your pain impairs your everyday activities and assist our team in developing a treatment plan that best suits your needs and lifestyle.
